This form is for collecting contact information and basic information to evaluate if they are a potentially suitable client for our care. If they are possibly suited based on location, due date, health, and surgical history then they may be offered a consultation or appointment based on their individual need and our availability. This information is never shared or sold to anyone or any company other than OMW. Forms are deleted if not used and shredded if printed and used. These potential clients may be contacted via phone call, text, or email by OMW team members only.
